Medication Monitoring & Reviews

A pharmacist will participate in the interdisciplinary review of patient medications.  

Cleary’s Pharmacy will organise & lead these reviews, carrying out resident audits and completing requisite reports, liaising with the care home team and relevant prescribers, thus reducing the administrative burden for Care Home staff.

Care Home Vaccinations

Cleary's Pharmacy provides onsite vaccination clinics to Care Homes.

A Pharmacist will take care of your Residents, Staff & Visitors, providing flu and/or covid vaccines in your Home at a time that suits you. 

Medicines Management Policies

A Pharmacist will be available to actively participate in the development of medicines management policies in the  Care Home and to advise prescribers and other members of the care team on the safe and rational use of medicines.

Delivery 7 days/week

Cleary's Pharmacy opens 7 days a week. We provide free delivery to our Care Homes every day. Your patients have fast, easy access to the medicines they need, when they need it.

Education

Our Pharmacist will provide Educational talks & information sessions for Residents, their visitors and Care Home staff. A pharmacist will visit regularly to give patients and their carers opportunity to discuss their medicines.
